There are two types:
- Acute radiation cystitis: Develops during or shortly after radiation therapy and may improve on its own.
- Delayed (late) radiation cystitis: Develops six months or longer after completing radiation. Sometimes years or even decades later. Late cystitis occurs because radiation causes progressive fibrosis and scar-like tissue buildup in the bladder wall. Late radiation cystitis is not rare. Anywhere between 23% and 80% of people who receive pelvic radiation develop some degree of cystitis, depending on dose, field, and technique. What Are the Symptoms of Radiation Cystitis?
Symptoms range from mild to severe and often worsen over time without treatment:
- Frequent urination and waking multiple times at night to urinate
- Urgent, uncontrollable need to urinate and incontinence
- Pain or burning sensation when urinating (dysuria)
- Bladder muscle spasms
- Reduced bladder capacity. Your bladder cannot hold as much urine
- Blood in your urine (hematuria), which defines hemorrhagic cystitis

What Causes Radiation Cystitis to Persist for Years?
Radiation therapy saves lives, but it also damages healthy microvasculature. This is the classic 3-H model of delayed radiation injury: hypovascular, hypocellular, and hypoxic tissue.
Radiation causes obliterative endarteritis. Small blood vessels narrow and close. The bladder wall becomes poorly perfused, fibrotic, and oxygen-starved. The fragile new vessels that do form bleed easily, which is why hematuria is the hallmark of chronic radiation cystitis.
